In the event of an emergency, dial 911 or go the nearest emergency room. Patients are asked to contact their Primary Care Manager (PCM) within one duty day to report any care received at an emergency room to ensure proper follow-up. To report, call the Appointment Call Center at 850-283-2778.

The 325th MDG provides medical care to more than 36,000 beneficiaries in northwest Florida and is fully accredited by numerous professional organizations, including the Accreditation Association for Ambulatory Health Care (AAAHC). Accreditation by AAAHC indicates the clinic meets or exceeds rigorous national standards used to measure excellence in medical care.

The 325th Medical Group earned "Excellent" ratings on both the 2011 Air Force Health Services Inspection and the Air Education and Training Command Compliance Inspections.

Beginning October 1, 2015, most companies who issue credit cards will have complied with the requirement to replace cards with chip enabled EMV (Europay, MasterCard and Visa) cards.  For those beneficiaries who are required to pay TRICARE Prime enrollment fees and who have elected to pay their enrollment fees on a recurring monthly basis via credit card this should not present a problem unless the company has elected to give them a new credit card number.  For those who are assigned a new credit card number and who have not updated their information with Humana, their October 1 payment will fail.  Humana has prepared for this possible occurrence and will initiate an outbound call to the beneficiary to enable them to update their credit card information over the phone so that they will not be dis-enrolled for non-payment, and there will be no lapse in their coverage.  The attached document is the scripting of the message the affected beneficiaries will receive, and they can, at any time, contact Humana at 800-444-5445, or go to www.humanamilitary.com to update their payment information.
